The Ontario Football Conference is committed to protecting your privacy while registered as a member of the Ontario Football Conference.
This Privacy Statement informs you about our practices relating to the collection, use and disclosure of personal information that may be received through your registration with the Ontario Football Conference and/or its affiliation with the, ontariotacklefootball.com (OTF), the Canadian Junior Football League (CJFL), Drug Free Sport Canada or any other organization that may otherwise be collected by us. By registering with us, you consent to the collection, use and disclosure of your personal information (as defined below) in accordance with the following terms and conditions.
This Privacy Statement also explains how you can contact us if you have a question about, want to make a change to, or delete, any personal information we may be holding about you. We strongly recommend that you take the time to read this Privacy Statement and retain it for your future reference.
Personal Information
For the purpose of this Privacy Statement, “personal information” means information about the identifiable individual.
What Personal Information We Collect
When you register with us we will collect personal information from you that we require in order to register you with both the OFC and CJFL and insure you with OTF during your participation with the Ontario Football Conference. The type of personal information we collect will depend on the nature of the information requested by the league, government agencies and insurance provider, but may include at least some of the following:
· Contact information (such as name, address, e-mail address and telephone number);
· Proof of identity (Ontario health number, driver’s license number, Birth Certificate);
· Emergency Contact information (such as parent); and
· Medical information
How We Use Your Personal Information
We use your personal information to:
- Register you with the OFC and CJFL and do all things necessary to administer those registrations;
- Communicate with third parties to ensure that you are properly insured in case of injury while participating in the Ontario Football Conference;
- Communicate with third parties on your behalf (such as health care providers) as required to effectively ensure your health if injured; and
- Develop and maintain our relationship with you and communicate with you.
When We Disclose Your Personal Information
We may share personal information with third parties we engage to facilitate the registration and to insure you so to allow you to participate in both the OFC and CJFL or to carry out one or more of the purposes described above. These other organizations are prohibited from using your personal information for any purpose other than to provide those services or duties outlined above and are required to protect personal information disclosed by us and to comply with the general privacy principles described in this Privacy Statement.
We may also disclose your personal information to third parties, such as those described above, as may be required to carry out the above described services. We disclose your personal information to such parties on an “as needed” basis only.
We reserve the right to disclose personal information to a third party if a law, regulation, search warrant, subpoena or court order legally requires or authorizes us to do so.
Knowledge and Consent
We collect personal information about you only when you voluntarily provide it. Typically, we will seek consent for the use or disclosure of your personal information at the time you register with us. In certain circumstances, consent may be sought after the information has been collected but before use (for example, when we want to use information for a purpose not covered by your initial consent i.e. to provide information to a college or professional scout).
Depending on the nature of the services requested, it may also be necessary for us to collect personal information about you from a third party (such as a governmental agency, family member, your doctor, OHIP, your employer, or a hospital where you received treatment. We obtain your consent to this collection at the time you register with us.
You may withdraw consent at any time, subject to legal or contractual restrictions and reasonable notice. If you wish to withdraw your consent at any time, please contact our Privacy Officer, in the manner set out below. We will inform you of the implications of withdrawing consent.
We will not, as a condition of the registration of you with our organization, require you to consent to the collection, use or disclosure of information beyond that required to fulfill the purposes for which the information is being provided.
How We Protect Your Personal Information
We make all commercially reasonable efforts to ensure that personal information collected from you is protected against loss and unauthorized access. This protection applies in relation to information stored in both electronic and hard copy form. Access to your personal information is restricted to selected individuals within The Ontario Football Conference. In addition, The Ontario Football Conference employs generally accepted information security techniques and access control procedures to protect personal information against loss and unauthorized access.
Unless we need to share your personal information as part of your registration with the OFC and CJFL, all information you give us will be kept strictly confidential. If you provide us with an e-mail address, you authorize us to send to you correspondence, documents and other information related to your participation with the OFC and the CJFL that may be protected by this Privacy Statement (or otherwise be categorized by you as confidential or private in nature) through the use of the Internet (and, particularly, e-mail) in an unencrypted condition and without any guarantee of security or protection from interception by a third party.
Retention of Your Personal Information
We retain your personal information for as long as necessary to fulfill the purpose(s) for which it was collected and to comply with applicable laws. In addition, your personal information may be retained in our alumni database in order to maintain our relationship with you and to communicate with you in the future,
